Mozaic Home Care HEADER

The clinical team of Mozaic Home Care has the expertise to help you achieve the best possible outcomes as you recover from a hospitalization, surgical procedure, a new diagnosis or to help manage a chronic condition. Our professional registered nurses and licensed therapists will provide the medical attention you need to support your health goals, achieve optimum health and prevent unnecessary hospitalization.

 

Short-term home health care services are provided via a referral from your hospital medical team or through a prescription from your own physician. The Mozaic Home Care team will work closely with your physician to ensure your care is well-coordinated and consistent with your medical plan until your recovery goals are met.

Your home health care is customized to your needs and may include:

  • Nurses to address your medical needs
  • Physical therapists to help with mobility and improve function
  • Occupational therapists to help you improve your ability to perform everyday activities
  • Speech-language pathologists to assist with speech, voice and swallowing
  • Medical social workers to link you with community resources
  • Certified Nursing Assistants/Home Health Aides to assist with personal care

Skilled home health clinicians provide a range of services including:

  • Education and assistance on managing your disease or condition
  • Assessment and monitoring of your condition
  • Progress reports to your physician
  • Medication management and education
  • Wound care and instruction
  • Nutritional evaluation and education
  • Assistance with mobility challenges/fall prevention
  • Caregiver education and support

Eligibility requirements exists for all services. Eligible care is generally covered by Medicare and private insurance.
