Mozaic Institute on Aging is a center of excellence providing education and research in the field of geriatric care. It is a consultative service committed to those who are living at home and concerned about changes in health. Our priority is to enhance the quality of life for those individuals living with memory disorders and their caregivers through coordinated medical, psychological and social interventions that lead to an all-inclusive plan of care.

ElderCare Navigators are available to discuss needs by appointment in-person or over the phone. Consultations will be scheduled at your convenience. Service agreements and terms will be reviewed.
Payment packages include single session planning meetings, hourly services, and monthly on-going assistance plans. We can assist in determining the most appropriate and cost-effective option for you.
